Name of Item : Reaper Mask or Grim Reaper's Mask(or Grim's mask because having an item named after me would be amazing)
Details : So you could do 2 things A, something like this with a hood, or B, which is basically just a mask that covers the front of the face shaped like a skull.

Description : Once you look into the Grim Reaper's eyes a chill will rattle throughout your body, and then the breathe will slowly escape from your lungs as the muscles in your body relax. Then you wake up, you are nothing, you can't see, hear, or feel anything, and that it's just eternal darkness all around. All of a sudden you realize, this is Death.
