Yea, certain words could be blacklisted if necessary and because I imagine the text line would be set in the item like sounds it would be visible and able to be changed by admins/smods if misuse occurs.

Well I was referring the item ( if it gets inplemented ) be made like a custom belt title.

I think this idea is well thought out, has no downside, and would be a nice little addition to the game.
